About the role:

• Full- time, Monday – Friday - State Traveler
• Schedule – 4 weeks on/1 week off
• Physician Assistant / Nurse Practitioner will work autonomously to see Medicare age patients in the home, performing annual comprehensive health assessments – 45-60-minute-long assessments
• Provider will be a state traveler – location varies across the state based on business needs

Background:

• Board Certified Family Nurse Practitioner / FNP, Acute Care Nurse Practitioner / ACNP, or Adult-Geriatric Nurse Practitioner / AGNP
• 3 + years of experience as a Nurse Practitioner required
